Send customers a form after they book
Forms after booking moves the questions that can wait off the booking page and into a short form. The form is linked in the confirmation email and is due a day before the visit.
Which plan
On every plan, Lite and Pro. Switch on Forms after booking in Toolkit first. Site & access needs it switched on too.
What customers see
The confirmation email links to a short form, with one section per pet, vehicle or person in the booking. If it isn't filled in, the customer gets up to two reminders. Returning customers see what they said last time and tap "Nothing's changed" or "Something's changed".

Set it up
Step 1: Switch on Forms after booking
Go to Toolkit and switch on Forms after booking. Then follow "Set it up in Questions" underneath it.

In Booking page › Questions, press Add a question, or press one under "Suggested for" your trade. Write the question and choose the Answer type: Short text, Long text, Yes or no, Choice or Date.
Step 3: Put it in the form
Under When, choose "In the form after booking". Under Asked for, pick the services it's for, or leave All services.
Step 4: Mark sensitive answers Private
Open More options and tick "Private: only you and admins can see it". A private answer is never in emails, notifications or calendar events, and it isn't saved to the profile.
Step 5: Set Required and Highlight
In More options, tick Required if the customer must answer. Tick "Highlight the answer on the fact sheet" for things you need to spot on the day. Press Add question.
Step 6: Put the questions in order
On the Questions page, use the up and down arrows on each question. Customers see the questions in this order, under "In the form after booking".
Step 7: See what your customer gets
The confirmation email links to the form, which is due a day before the visit. Returning customers see their last answers and tap "Nothing's changed" or "Something's changed".

How different trades use it
- Dog groomers
- "Vet name and phone", "Allergies or medical conditions" and "Temperament and handling notes". The last two are highlighted on the fact sheet.
- Car detailers
- "Where will the vehicle be?" is asked for every booking, because the answer can change each time.
- Cleaners
- "Pets in the home" is asked once and remembered for the property.
- Massage therapists
- "Injuries, conditions or medication" is marked Private, so only the owner and admins see the answer.
- Podiatrists
- "GP surgery" is remembered. "Diabetes, circulation problems or blood-thinning medication" is marked Private.
Common questions
When is the form due, and are customers reminded?
The form is due a day before the visit. Customers who haven't filled it in get up to two reminders.
Who can see a Private answer?
Only the owner and admins. It's never in emails, notifications or calendar events, and it isn't saved to the customer's profile.
Do returning customers fill the whole form in again?
No. They see what they said last time and tap "Nothing's changed", or "Something's changed" to update it.
Can a question that changes the price go in the form?
No. Questions that change the time or price are asked when booking, so the customer sees the result before choosing a time.
Should I keep clinical records in the form?
No. The form is for the practical details you need before a visit. Clinical records belong in your own clinical system.
Is Forms after booking on the free plan?
Yes. It's on every plan, Lite and Pro.
